At Trinity House Care Home, we are pleased to offer Dementia Care that is individually tailored to each resident. We take pride in standing out for several reasons, especially due to our home manager, Ann. With her expertise, knowledge and specialised training in dementia care, Ann leads our highly trained Dementia Care team. Every member of our team has received professional training to handle all aspects of dementia with skill and compassion. At Trinity House Care Home, we reject the idea of a separate dementia unit. Instead, we foster an inclusive, stigma-free environment where everyone is equal and safe.

What is Dementia Care?

Dementia care is specialised care and support provided to individuals living with dementia within a residential facility. Dementia is a progressive condition that affects cognitive functioning, memory, behaviour, and overall daily functioning. A specific dementia care floor or unit in a care home is designed to provide a safe and supportive environment for individuals with dementia, ensuring their well-being and quality of life.

The aim of dementia care is to help residents preserve their sense of self and independence for as long as possible. No two people with dementia are alike as they will be at different stages of their dementia journey and it will progress at different rates, that’s why each dementia resident will receive a personalised care plan designed to meet their unique needs which will be regularly monitored.

Dementia Care at Trinity House Care Home

Our approach to care is tailored to each individual. Our main objective is to ensure each resident feel at ease and safe in their new home, and to provide them with whatever they desire. We work hard in order for residents to achieve some form of independence, and our skilled staff is committed to providing the best person-centred care possible.

Our Care Teams and Activity Coordinators go above and beyond, talking with our dementia residents and their loved ones to identify activities and therapies that are appropriate for their specific needs in order to fully understand what is required. It can be difficult to know what to expect from dementia care in a care home, but we are here to reassure you that it can change someone’s life for the better. Trinity House Care Home also has an open-door policy where loved ones can visit at any time of day.

Before residents move into Trinity House Care Home, we arrange a consultation with our Home Manager, the prospective resident, and their family members. This crucial step allows us to assess and understand their individual needs. We have an open collaboration, working closely with residents to grasp their daily routines, interests, and preferences. These insights are then incorporated into their personalised care plans. Our approach to care is person-centered as we are dedicated to supporting residents in maintaining their independence and dignity.

We are committed to investing in the training and development of our staff. Specifically, our dementia care team receives ongoing training to ensure they remain well-informed about the latest advancements in dementia care and support. With a wealth of experience and expertise, our teams are well-equipped to deliver high-quality care to our residents with dementia. This includes providing support with various aspects, such as engaging activities, therapeutic interventions, effective communication, mobility assistance, medication management and personal care.

Trinity House Care Home offers a diverse activities programme for all residents, featuring a mix of both group and one-on-one activities. Our offerings include regular arts and crafts sessions, music therapy, reminiscence activities, and a variety of other options. These activities and therapies are thoughtfully selected to enhance the physical, mental, and emotional well-being of all our residents, including those living with dementia.
