Introducing ‘Nourish’ at Trinity House Care Home

Offering the very best in person-centred care for our residents is our main priority and we are delighted to announce that we are partnering with Nourish the award-winning digital care management system at Trinity House Care Home.

From our initial care assessment prior to moving into the home and throughout a resident's time with us, we customise care plans for every resident which are then recorded on Nourish. Implementing this system empowers our care teams by using data to understand even better each of our residents. 

Nourish provides insights to our team that allow for better decision-making and greater visibility for monitoring, analysing, and improving care practices to ensure that personalised care is at the heart of everything we do.

Via handsets, that look like mobile phones, our care team is able to accurately record at the point of care, which in turn enables them to spend more quality time with our residents as opposed to having to manually update care plans.

Not only does this allow information to be recorded timely and accurately, it also provides a further level of assurance for their loved ones of the care and support that residents are receiving.
